Wellness Center Policies
 
Booking

Please book your appointment as far in advance of your preferred appointment date as possible to ensure your first choice of appointment time and therapist. You may book appointments in person or by phone. Group appointments must be secured by credit card or deposit. We attempt to confirm all appointments in advance. If you must cancel or reschedule, we ask that you do so 24 hours or more before your scheduled appointment date. We reserve the right to require payment for “no show” appointments.

Arrival

Please arrive 15 minutes prior to your appointment to allow a relaxed check-in and to fill out required paperwork (medical history, consent/waiver forms and other necessary intake paperwork). All information is confidential and is collected solely to provide you with the best care possible.

Medical Conditions

Quality service is our top priority. Please advise your service provider of any medical conditions, use of medications, and concerns you may have. Please tell us if you are pregnant, attempting to conceive, have high blood pressure or other circulatory issues, or other health issues that might affect your therapy. The more we know about your health status, the better we can serve you and the more effective your therapy will likely be.

Payment for Services

Payment is due at the time of service. We accept cash; local checks; American Express, Visa or MasterCard debit or credit cards; and Integrative Life Solutions gift cards. There is a $35 fee for a returned check. A gratuity is not automatically added to your service fee and is left to your discretion. Please leave cash gratuities with the front desk staff in an envelope that will be provided upon request, or you may add a gratuity on your credit card charge. All therapy and service fees are subject to change without notice.

Personal Assistance

If you become uncomfortable or need specific attention at any point during your time with us, particularly during a therapy session, please notify the service provider or any of our staff members immediately. No one, least of all your service provider, wants you to feel uncomfortable. We try very hard to anticipate your needs, and we invite you to let us know your personal preferences.

Confidentiality

Any personal information Integrative Life Solutions, Inc., may need to collect from you when you use our Wellness Center services or purchase products from us, either in person, online, by phone or by mail, is kept strictly confidential. With the exceptions of sharing necessary information to process your credit or debit card so that we may fulfill your order or to ship orders to you via common carriers and the Post Office, we will not share, sell or disclose your private information to any other entity for any reason other than those required by law or in response to your written authorization. When paying by credit card, you will be required to provide us with the three-digit CVV2 security code from the back of your credit card. This code is necessary so that our merchant bankers can verify that the sale to be processed is in fact at the request of the card owner or owner’s authorized agent. This is for both your protection and ours.
